I think the organizers are largely responsible for not doing something about it before something serious happened. It was entirely predictable.

The Tour covers several thousand kilometres of racing on public roads over three weeks or so. The organisers can’t fence off or police the verges and roadsides of any given day’s race route and they can’t stop spectators coming out in their tens of thousands to watch the Tour riders go by. That’s just how it is.
