It didn’t merely occur, it was rather the rule, especially the estimates of the numbers of victims in particular camps and extermination sites. But I think this always happens. During the Serbian genocide there were some very lurid stories about people killed by the Serbs en masse and then burned in ovens at some factory - the story was later debunked by the official commission which investigated it.
